Thyroid Questions with Dr. Rosensweet, MD - April 11, 2023

Key Timestamps

00:42 - I'm not sure if he specializes in this area, but could he discuss thyroid issues and his treatment approach? I have had thyroid issues for a long time and have been given Synthroid, and I don't feel like it does too much for me. I guess I am wondering if he has an "approach" or protocol for treating thyroid?

4:22 - Dr. Rosensweet explains functional hypothyroidism

6:46 - Dr. Rosensweet discusses bioidentical T4 and T3 thyroid hormones, as well as desiccated thyroid hormone

10:22 - Why Dr. R decided to use the process of hormone titration when treating women with hypothyroidism

12:24 - Dr. R shares how he assesses thyroid function, including the labs he uses to assess thyroid function. He also discusses his hormone titration dosing approach and why he uses long-acting T3 vs. something like Cytomel

20:48 - Why it is so important to use hormone titration to determine optimal hormone dosing, and not just give a standard thyroid dose to all women

22:22 - When should a woman retest her thyroid levels once she has found her optimal dose using hormone titration/symptom alleviation?

30:00 - Do you find that thyroid issues become more problematic during perimenopause when women’s hormones are changing?

31:51 - For women who suffer with genitourinary symptoms (i.e., vaginal dryness, discomfort/pain with intercourse, etc.), do you recommend starting with a local estrogen or will systemic estrogen therapy improve symptoms on its own?

39:25 - Just a reminder about the importance of using vaginal friendly oils, creams, or gels if using HRT vaginally; some oils, creams, or gels can irritate the vaginal tissue

40:37 - This is a follow-up question to the DIM question last week for a member who has been struggling her whole life with high estrogen. She is 46, has the MTHFR gene, and has tried a million tools to detox properly. She started DIM Detox with Pure Encapsulations a few weeks ago. The formulation contains other detoxing properties like NAC and milk thistle, and I am feeling an improvement. While she is not “hanging her hat” on this, she feels like we also can’t rule out that DIM, with the other proper strategies, can be helpful. She won’t be on it long term, but wanted to run this by him.
